Case Management and Care Coordination in North Carolina: Preliminary Recommendations

Abstract

This report, prepared by the Center for Aging Research and Educational Services at the University of North Carolina-Chapel Hill, presents the leading principles and program recommendations which are believed to underlie the development of a coordinated, case-managed system of long-term care for the elderly in North Carolina. The principles were developed by examining the experiences of other states and are reflective of the collective opinions of key individuals spread throughout North Carolina’s long-term care network.
