The policy forum of this issue focuses on transitions of care in the health care delivery system. Transition practices to and from hospitals, nursing facilities, and adult care homes are explored, and among other topics, select commentaries focus on community engagement, transitions for special populations, and Community Care of North Carolina’s transitions program.
